Re-Entry and Navigation

       The Re-entry and Navigation program provides short term case management to individuals, referred by Department of Corrections, who are returning to the community after a period of incarceration. The services provided through this program include referrals to relevant service agencies as well as identifying needs and opportunities to improve life skills. Reentry navigation aims to support individuals in the first few months after their release to ensure smooth and successful community reintegration. 

 

What participants can expect from participation in the program:

    • Choose whether to participate in Re-entry and Navigation – participation is voluntary
    • Admit and take responsibility for actions
    • Establish and maintain clear expectations for healthy and positive community behaviors
    • Identify needs and create service plan for re-entry
